Obtained device tokens for the same APNs from multiple IDFVs

The device token is supposed to be a unique ID, Is there a possibility that the above-mentioned events will occur due to the specifications? What are the possible cases when it occurs?
Example) Model change, OS update, application reinstallation, etc.
Also, have you reported similar issues?
Any advice on how to fix this?
