We are seeing some inconsistent behavior with regards to the on-demand VPN. Is there a possibility that the system will not turn on the on-demand VPN if it keeps failing to connect or hasn't been used in a while? Also, could on-demand settings be cached somewhere on the VPN profile and not be updated as expected?

I am not aware of anything like this. If you find that you are stuck in a loop where your On-Demand rules are not being executed I would cycle your network interfaces and then your VPN configurations to see if that has an affect.

The best way to check your settings is on the VPN configuration itself in the Settings app. This should be an accurate representation of what is being used for your VPN.
