Error trying to request 1-on-1 lab appointment

I'm trying to register for the SwiftUI lab at this link, but I keep getting an error on submission saying "There was a problem processing your request."

No field seems to be missing or marked red. Any clue on what it could be? Thanks!

Hello Stanis, were you able to submit the lab request today? If you haven't tried, we encourage you to try again. If you continue to get the same error message, please contact wwdc@apple.com with your information and screenshot of the error so we can help you troubleshoot this issue.
