do my customers have to use Apple Business Manager to download my app?

instead of registering a Org ID to push my custom app to a business customer, can I invite them by Apple ID instead

when I revoke access from a user, can I do it from App Store Connect instead of the customer revoking from ABM?

after revoking access, is the Custom App immediately disabled from the user's phone or does it require the user to delete the Custom App first?

Yes, the only way for users to get access to a Custom App is via Apple Business Manager. The business customer can purchase licenses for your app and distribute them with MDM (works in Apple School Manager as well), or purchase redemption codes and distribute them to individual users (Apple Business Manager only).

You cannot revoke access to a redemption code for a user, and the business customer would be responsible for revoking a license from a particular device. The only way to prevent a business customer from purchasing more licenses of your app would be to remove their organization ID from App Store Connect.
