Provisioning Profile stopped working?

We have been developing a multi-platform app for a few months now. We have had the app working on iPhone for less than a month including provisioning. Starting December 1st we can no longer deploy to iPhones. I get two errors depending on the phone i'm trying to deploy to. (e8008015 / e8008016) No changes to provisioning were made up to that point. This led me to try and reinstall / update the profiles on one of the iPhones. I removed the profile, then I followed the same steps as before. When downloading the profile to the iPhone, it takes me to the "VPN & Device Management" section of Settings. In the past, this is where you could review / install the downloaded profile. It no longer has any provisioning information. There is no provisioning section in the phone settings either.

software/hardware in use: Win10 machines running Delphi 10.4, PA Server 21.0, Xcode 13 (13A233) on a iMac (Retina 4k, 2017) BigSur 11.6, iPhone SE ios 15.1
