While trying to Upload to App Store via XCode, I've been getting:

"iTunes Store Operation Failed. This action could not be completed. Try again."

for about 1-2 weeks. I've been using the old Application Loader in the meantime to upload to Testflight until today. Where the Application Loader began giving me:

"Exception occured when creating MZContentProviderUpload for provider. (1004)"

I have absolutely no way to get my build up to TestFlight and the app store now. Any insight into this would be lifesaving.

I've been trying to upload a tvOS app to the store and have been running into issues ALL DAY.

The most recent issue I've run into is the same one you're experiencing:

"Exception occurred when creating MZContentProviderUpload for provider. (1004)"

This error occurs when I try to submit via Application Loader 3.4.

MZContentProviderUpload is solved but I still haven't been able to submit through XCode for weeks. Still not working. Has me pretty spooked that I have to use the Application Loader like I'm stuck in 2009.
