healthcare app in app purchases apply?

hi i have been searching the forums and also the web but I cant find a diffinative answer.

1. We are building a healthcare app where physios will be giving advice to clients via video calls and messaging will this type of business require us using the in app purchaces via apple?

I read that a Chinese company did not have to pay the apple tax but im really non the wiser.

2. If we do have to use apples in app payments do you know of a way we can pay the physios like we do on our web app via stripe connect where payments are automatically split between us and the pysios?
