The problem occurs when using XCode (v11) on a Mac to build and run the Unity project (built for iOS) on a connected iPhone.
All XCode settings are the same as our uploaded project. The app works fine when the iPhone downloads it through TestFlight.
iPhone is connected via cable. Build target is set to iPhone, and I select the "Build and Run" button. The build succeeds (we even get the "Build Successful!" icon).
The progress indicator then says "Running <AppName> on iPhone", but an error window immediately pops up that says "iPhone not available" and "please reconnect the device".
We have had this result using two different phones. Restarting XCode, restarting the Mac, and restarting the phone do not correct it.
