Has anyone else noticed that negative keywords aren't working, either broad or exact match, campaign or ad group level?
I've mostly noticed it when keywords have dashes or other symbols in the negative.

Solved. You have to exclude symbols in your negative keywords. Per Apple's CSR team:
"Thank you for your patience with us during this process.We’ve reviewed the behavior with the negative keyword, ‘brand – tagline’. Our Technical Support team has determined that to ensure that this negative keyword does not deliver any impressions, please add this negative keyword to your campaign and ad group without the dash ‘-‘ as an exact match negative keyword. See below.
