Please provide evidence why CPF, phone number, and date of birth are required upon registration.

Please also provide evidence of your relationship with all government entities, city halls, etc

We still need to provide this information if we provide evidence of relationship with our customers? That's normally what government entities asks from citizens to identify them, what kind of legal document is needed to address this so I can get in touch with the right people?

What is CPF ?

That's normally what government entities asks from citizens

Are you such an entity ? Otherwise, that's why they ask you. They probably want to check your app is not just for collecting such information but has a real need of it.

What kind of evidence is acceptable

A letter from them acknowledging this relationship would certainly be helpful here. Or some published information on such relationship.
