NFC PassKit Certificate request form submits without confirmation

I’m trying to request an NFC PassKit Certificate through https://developer.apple.com/contact/passkit/. After clicking Send, the completed form is POSTed successfully and receives 200 OK, but the server returns the original form instead of a confirmation page. The page’s passkit.js then clears all fields, and Developer Support confirmed that my earlier submission was never received. Has anyone else encountered this behavior or found another way to submit the NFC PassKit Certificate request?

@jboddiford @mannafanna I received an email earlier today from Apple Developer Support. They said they believe the issue has been resolved, so I went ahead and re-submitted my application. After submitting, I was shown "Thanks for contacting us. We will review your information," and I received a confirmation email, so I think the form is now good-to-go.

Hi, I just experienced the same issue. I submitted 3 times thinking maybe my adblocker was causing problems, but no difference when i disabled it. I just opened a ticket with support to see what they can do for me. Were you able to get the certificate issued? What did you do?

Hi, I've gone back and forth with Apple support for the past 7 weeks. They finally elevated my ticket 2 weeks ago to the engineering team who owns that form, but I haven't heard back from them since.

Hi, we're hitting the exact same issue, submitted the NFC PassKit Certificate request multiple times over the past couple of weeks across different browsers and devices, always returns to a blank form with no confirmation and no email. We also opened a Developer Support case but haven't had a substantive reply yet. Did engineering ever get back to you after the escalation? Would appreciate knowing if you find a way through this.

Thanks for tagging me and for your persistence to get support to fix this! I went back and just re-submitted my form as well. Good Luck and please tag me in a future update when you get your certificate (or not) as I'm unsure how long this process will take and curious how others experience goes!
