I know that it is not acceptable to sell digital content, access to full-version and so on from external places. But what if it's a service? For example, I want a fitness app, with a remote mentor (instructor) service. If you buy access, your mentor will give you recomendations, give you tasks and challenges and so on. You cannot use app without access. Can I sell access from website in this case? Or such an app will be rejected?

Might want to be aware of the following review guideline:
Your app should include features, content, and UI that elevate it beyond a repackaged website. If your app is not particularly useful, unique, or “app-like,” it doesn’t belong on the App Store.
There is a set of guidelines for App Review. Check them, especially section 3. Use of IAP is required to unlock code within the machine but is not required to sell services through an app - which is what you seem to be suggesting.
One issue, alluded to by KMT, is that all apps have to do something without requiring extra purchases. So your app will, at a minimum before a purchase is made, have to explain the services that are available and how great they are.
Let's just say that the app will have description and explaination of what services will be available after purchase and how great they are. But without buying access app won't have any other functionality. Do I need to manage such purchase through iAP or it will be okay to lead users to a website, where they can buy access?

Since we're repeating, see:

You can't sell an app via IAP. You can't force users to pay on a website just to use the entire app.
Unless you change your plan, IAP ok or not, it would be foolish to waste your time otherwise, because while you are free to ignore us, you can't ignore app review. Better you learn that here, now, than later.
